This error suggests that on the return you have crossed the exempt or deferred box for Class 4 nic on more than one set of supplementary pages.

If you have multiple sets of the same source of income or multiple sources of income where class 4 nic is applicable then you just need to cross the deferment or exempt box on one set of pages, HMRC advise the first available set.
